Publicity, Testimonial, and Likeness Release ("Release")

By submitting information on the JUUL Community site, I authorize and consent to the use of my: (i) name (or any part thereof); (ii) city and state of residence; (iii) photograph, video, image, likeness, and voice; (iv) posting, endorsement, or testimonial (collectively, "Reviews"); and/or (v) any information I have submitted to JUUL Labs, Inc. (JLI) in any form (collectively, "Materials") to appear in any media released by JLI, by those acting under JLI's authority, or by their respective agents, representatives and/or assignees (collectively "those acting with JLI's authority"), in connection with any advertisement or promotion of, communications about, and/or advocacy regarding JUUL products, services, and/or their use, and/or for any other lawful purpose, including but not limited to communications with the public, advocacy groups, policymakers, and members of the media.

I also irrevocably grant to JLI, and those acting with JLI's authority, the unrestricted, absolute, perpetual, royalty-free, irrevocable, transferrable, worldwide right and license to use the Materials without further consideration or notice to me or any third party, including without limitation to reproduce, copy, modify, create derivative works of, display, distribute, transmit or broadcast, or otherwise use and permit to be used the Materials or any part thereof (including with or without my name or other biographical information), whether alone or in combination with any other materials, without any further review, approval or additional permission from or attribution to me, in connection with any advertising, promotion, publicity, advocacy, or commercial activities, media, or materials. I agree that JLI owns all rights, including the copyright, in and to any works that include or incorporate the Materials made by JLI or those acting with JLI's authority, and that JLI may register the copyright in such works in its own name or such other name as JLI may, in its sole discretion, deem appropriate.

I understand that JLI reserves the right not to use any Materials and to remove or withdraw them at any time and for any reason. I also understand that this Release supersedes the JLI Privacy Policy with regard to the disclosure of information to third parties for the purposes described herein.

I certify that: (i) any Reviews or other Material I have submitted are true and accurate and reflect my personal experience and honest beliefs; (ii) I am a user of the products I review on this site; (iii) I have not been given remuneration for submitting any Materials; (iv) the Materials I submitted only identify me, and I have all necessary rights to grant the aforementioned permissions, as well as to grant JLI the waivers, consents, licenses, authorizations, releases, and rights ("Rights") herein.

I represent and affirm that I have the authority to grant the Rights set forth in this Release, that I have not previously sold or assigned the same, and that no other party has any rights in or to any of the Materials. I also certify that I am over the age of 21 years and competent to contract in my own name, and that I have read this Release and fully understand its contents.

CAMPAIGN: UK Share Your Story User Generated Content Campaign

COMPANY: Juul Labs UK Ltd. ("Company")

CONTRIBUTION: Providing a testimonial of your experience of using Juul Labs' products.

Any content, including without limitation any photos, reviews, messages, quotes, which I create, conceive or provide as part of my Contribution or is created by the Company or any of the Company's agents, affiliates or suppliers in connection with the Contribution shall be referred to as the "Content".

1. I hereby agree that the copyright (if any) and all other rights title and interest in and to my Contribution in the Content shall vest in and is hereby irrevocably assigned to the Company and its group companies (and this assignment shall operate to the extent necessary as a present assignment of future copyright) and the Company and its group companies shall have the unfettered right to deal with the Content containing such Contribution or any part of it in any way that it thinks fit. Accordingly, I agree that the Content incorporating my Contribution or any part of it may be exhibited or otherwise howsoever exploited (and such exploitation may include the exploitation of ancillary rights therein) in all media and formats throughout the universe for the full period of copyright and all renewals and extensions thereof and thereafter so far as may be possible in perpetuity without acknowledgement to me. I agree that the Company shall be under no obligation to include all or any of my Contribution in the Content.

2. I hereby waive any so-called "moral rights of authors" in the Content and such rights under the Copyright Designs and Patents Act (the "Act") or under any statute or common law whether now or hereafter brought into force in any part of the world as I now have or hereafter acquires in relation to the Content.

3. I hereby grant the Company all consents under the Act to enable the Company to make the fullest possible use of my services hereunder.

4. The Company shall not be liable to me or my legal representatives for any loss or damage or injury to me or my property cause or suffered in connection with the Company's making of the Content unless caused by the negligence of the Company and recoverable on that ground. Further, the Company shall be released from any liability including any damage or harm suffered by me in connection with or as a consequence of my Contribution to the Content. For the avoidance of doubt, nothing in this release form shall limit or exclude the Company's liability (i) for death or personal injury caused by its negligence; (ii) arising out of fraud or fraudulent misrepresentation; or (iii) for any liability which cannot be excluded or limited by law.

5. I hereby agree that I am the sole owner of my Contribution with the ability to assign all rights herein, and that my Contribution shall not contain anything which is an infringement of copyright or which shall bring the Company, or a broadcaster into disrepute or which is defamatory, blasphemous or obscene and that the contents of my Contribution shall be true in all respects to the best of my knowledge and belief.

6. I understand that the Content may include personal data which may identify me and in which my location at the time of capture may be identifiable (together, my "Data"), in relation to which the Company shall act as controller. I agree that the Company may process and store my Data pursuant to and in accordance with all applicable laws and regulations relating to the processing of personal data and privacy (including the EU General Data Protection Regulation (EU) 2016/679 ("GDPR") and the Data Protection Act 2018) for use in connection with the Content in relation to the Campaign. The Company may transfer such Data to its group companies and its appointed agents (in some cases transferring and storing Data outside of the European Economic Area, but always in line with applicable laws and regulations). I confirm that I have read and understood the additional information regarding how the Company processes my Data in its Privacy Policy (https://www.juul.co.uk/privacy-policy). I understand that I have the right to access, amend and delete any data relating to me, and can ask the Company to stop processing my Data at any time notwithstanding the other provisions of this release form (though this will not affect the lawfulness of any processing carried out by the Company before my request). I expressly agree and understand that personnel from the Company, appointed agents and/or group companies may contact me directly to discuss the Campaign and my contribution utilising the contact details on this form.

7. I agree to treat my Contribution and my services under this agreement and anything I learn about the Company whilst carrying out my services under this agreement as confidential and shall not disclose the same to any third party.

8. This agreement may be assigned to the Company's successors, licensees, assignees and group companies.

9. This agreement is governed by English Law and subject to the jurisdiction of the English Courts.
